The Community Wellness Fund.

 

Heidi is the Hub Director for Pathways to Wellness in the Ely area, and facilitates a network of area professionals to identify and find solutions for gaps in our continuum of care. Pathways to Wellness is one of those solutions, offering education and care facilitation to connect individuals and families to services and supports that meet their health and wellness goals and needs. Heidi will share about Pathways to Wellness and another strategy, the community wellness fund that is important in addressing barriers to success.
